Ithaca pays the most at $151,780, then Syracuse at $149,510, Watertown-Fort Drum at $143,250, Elmira at $142,640 and Kingston at $142,360. Not one of them is downstate. Upstate hospitals and independent pharmacies pay a recruitment premium that the New York City market, with its dense supply of pharmacists and pharmacy schools, does not need to.

Graduate from an ACPE-accredited PharmD programme, complete the required intern hours, pass the NAPLEX and the New York jurisprudence examination, and apply to the New York State Education Department, Office of the Professions. New York licenses pharmacists through the education department rather than a health agency, and its continuing education requirements are administered the same way.

Because supply is genuinely thin there. Hospitals and independent pharmacies in those markets have to pay above the state median to fill posts, and the premium persists year to year because the underlying supply problem does not resolve.

Mainly a different administrative path rather than a different standard: applications, registration renewal and continuing education all run through the Office of the Professions rather than a health department.
