Hands-on personal care in the client's home, under a registered nurse's plan of care: bathing, dressing, grooming, toileting, transfers and ambulation, meal preparation, light housekeeping, medication reminders and basic vital signs. What they do not do is nursing judgment — no wound care decisions, no dose changes, no assessment calls. The most valuable thing an experienced aide contributes is noticing the change (new confusion, a weight gain, a skin breakdown) and reporting it up in time.

Federal regulation requires a minimum of 75 hours of training, of which at least 16 must be supervised practical instruction, for aides working at Medicare- and Medicaid-certified home health agencies, followed by a competency evaluation. Many states set a higher bar, and some agencies run their own longer programs. The BLS OEWS May 2025 median for Home Health and Personal Care Aides (SOC 31-1120) is $35,800 a year, or $17.21 an hour, with the 10th percentile at $27,040 and the 90th at $45,040. Two things distort that figure: it is a combined group that includes personal care aides doing no clinical tasks, and a large share of the workforce is part-time, so the annual number reflects hours as much as hourly rate.

It is an excellent entry point and a difficult long-term destination. Entry costs weeks rather than years, hiring is close to guaranteed at roughly 765,800 openings annually, and the work is genuinely meaningful. Against that: pay tops out around $45,040, hours are often part-time and unpredictable, mileage is frequently unreimbursed, and the physical injury risk from transfers is real. Aides who plan a ladder — CNA, then LPN or RN — get the most out of it.

Supervision and scope. A home health aide works under a nurse's plan of care for a certified agency, performs hands-on personal care and documents clinical observations, and must meet the federal training and competency requirements. A personal care aide or companion may provide only housekeeping, errands, meals and company, often with minimal formal training. BLS combines both into SOC 31-1120, which is exactly why its $35,800 median sits below what trained agency aides typically earn.

Generally no. Aides may remind a client to take medications, hand them a pre-filled container, open packaging and observe that the dose was taken, but administering medication — including injections, and often even setting up a pill organizer — is a licensed nursing task in most states. The rules vary, and some states permit trained medication aides under specific delegation statutes, so the operative authority is your state's nurse practice act and your agency's policy.
