In practice yes. Every state regulates social work titles, and hospital positions almost universally require at least master's-level licensure (commonly LMSW), obtained through the ASWB Masters examination after a CSWE-accredited MSW. The clinical license — LCSW or the state equivalent — requires about 3,000 hours of supervised post-master's practice and the ASWB Clinical examination, and is what permits independent diagnosis, psychotherapy and clinical supervision of others.

BLS reports Healthcare Social Workers (SOC 21-1022) at a May 2025 median of $67,880 a year, $32.63 an hour, from $46,220 at the 10th percentile to $100,360 at the 90th. Position within that band tracks licensure and setting closely: hospital and academic medical center roles with an LCSW sit well above the median, while entry-level and community nonprofit positions cluster near the bottom decile. Government and hospital employers generally out-pay outpatient and home-based agencies.

In many hospitals they are two roles on one team. Case management is often nurse-staffed and focuses on utilization review, medical necessity, insurance authorization and length of stay. Medical social work owns the psychosocial side — family capacity, mental health, substance use, housing, abuse, guardianship, end-of-life decisions — and the placements those complicate. Smaller hospitals frequently merge the roles into a single position, which is why postings use the titles loosely.

It is, in a way distinct from clinical burnout: the difficulty is usually structural rather than tragic.
