California physical therapists earn a median $124,190 a year, $59.71 an hour, per BLS OEWS May 2025, the highest of any state. The middle half earns between $100,790 and $141,260, and the full range is $85,580 to $163,960. Even the state 10th percentile is above the national 25th percentile for the profession.

San Francisco-Oakland-Fremont leads at $141,980, then Vallejo at $141,530, Santa Rosa-Petaluma at $140,100, San Jose-Sunnyvale-Santa Clara at $139,000 and Napa at $138,540. The top five are all northern California, and the north-coast and Vallejo figures matter most practically because they pay Bay Area rates without Bay Area housing costs.

Graduate from a CAPTE-accredited DPT programme, pass the National Physical Therapy Examination, and apply to the Physical Therapy Board of California, which also requires a California Law Examination and a criminal background check. Therapists licensed in another state apply by endorsement but still take the California law exam.

Home health and per-visit work sit at the top for clinicians, followed by skilled nursing, then acute care, with outpatient orthopaedics generally the lowest of the main settings. Directors and travel contracts sit above all of them. That ordering is more or less the inverse of how new graduates rank the settings by desirability, which is why the pay gap persists.

In the inland and north-coast metros, comfortably. Vallejo at $141,530 and Santa Rosa-Petaluma at $140,100 pay within about two thousand dollars of San Francisco-Oakland-Fremont on much lower housing costs. In San Francisco and San Jose the nominal advantage over the rest of the country is largely absorbed by rent, though it still beats most states on a net basis.

A combination of a large ageing population, a dense skilled nursing and home health sector governed by staffing requirements, and a licence that is slower to obtain than in most states because of the additional California law examination. Supply is constrained at the state boundary in a way that it is not for occupations with straightforward endorsement, and demand is not.

The relevant number is the state 25th percentile of $100,790, not the median, because that is roughly where a first or second licensed year in California lands. Against a typical DPT debt load that ratio is manageable in a way it is not in states where the 25th percentile sits in the eighties, which is the strongest single argument for starting a PT career in California.

Productivity expectations and documentation load, particularly in outpatient orthopaedics, where the pay is lowest and the visit volume highest. The settings that pay at the 75th percentile — home health and skilled nursing — pay more partly because they are harder work in ways that do not show in a wage table: driving time, case complexity and regulatory documentation.
