They triage, stabilize and disposition. Triage means assigning each arriving patient an Emergency Severity Index acuity level and deciding who cannot wait. In the treatment area the ED nurse works up undifferentiated complaints — starting lines and labs, running protocols for stroke, sepsis and STEMI, medicating pain, restraining and de-escalating when needed — then either discharges the patient or gets them upstairs. Increasingly they also care for admitted patients boarding in ED beds for hours because no inpatient bed is free.

Finish an ADN or BSN, pass the NCLEX-RN and get a state license, then enter the department either through a new-graduate emergency nurse residency or by transferring after a year or two on a med-surg or telemetry floor. Orientation runs roughly three to six months with a preceptor across triage, the trauma bay and fast track. ACLS and PALS come immediately; ENA's TNCC and ENPC courses usually within the first year.

No certification is legally required beyond the RN license, but the practical floor is higher than the legal one. Departments mandate BLS, ACLS and PALS, and trauma centers commonly require TNCC. The optional credential that matters for career progression is the CEN from the Board of Certification for Emergency Nursing, which typically expects about two years of emergency practice and unlocks clinical-ladder tiers, certification differentials and transport roles.

It is difficult in specific, nameable ways: you carry several patients whose diagnoses are unknown, you are interrupted constantly, you absorb violence and verbal abuse at rates higher than most nursing specialties, and boarding means the department is often holding inpatients it cannot properly staff. What nurses stay for is the inverse — no long-term relationship management, a defined shift, high procedural skill, and genuinely consequential minutes.

Breadth against depth. The emergency nurse handles many patients at once, briefly, without knowing the diagnosis, and optimizes for sorting and stabilizing. The ICU nurse takes one or two patients for a full shift with an established diagnosis and titrates them hour by hour. Both sit inside SOC 29-1141 at the $97,550 median, and nurses move between them — though the mental habits are different enough that the transfer takes a real orientation.

BLS reports no separate ED wage; emergency nurses appear in Registered Nurses (SOC 29-1141), whose May 2025 median is $97,550 a year, or $46.90 an hour. New graduates start nearer the $68,940 10th percentile, experienced certified nurses at busy trauma centers reach the $112,350 75th percentile, and the $137,470 90th percentile generally reflects high-cost metros, heavy overtime, or the transport and leadership roles emergency experience leads to.
