In sequence: earn the RN license (BSN preferred), spend three to five years in an ICU, emergency department or trauma center, collect the certification stack — ACLS, PALS, TNCC, often NRP, ideally CCRN or CEN — then apply to an air-medical program. Hired crews complete a flight academy covering altitude physiology, aircraft safety and survival training, and sit BCEN's CFRN exam, which most programs require within the first one to two years.

Because programs hire only seasoned ICU/ED nurses and layer call pay, night differentials and 24-hour shift structures onto base salary, working flight crews generally land in the upper half of that distribution, with chief flight nurse and program-manager roles at the top of it.

Three years of recent ICU or emergency experience is the near-universal program minimum; five years — ideally at a trauma center, with transport or charge experience — is what competitive applicants actually bring. The requirement is functional, not bureaucratic: aboard the aircraft the nurse manages ventilators, RSI and blood products with one partner and no backup, so programs screen for nurses who have already run high-acuity emergencies independently.

The defining credential is BCEN's Certified Flight Registered Nurse (CFRN), typically required within one to two years of hire. Before that, programs expect ACLS, PALS and a trauma course such as TNCC, frequently NRP for neonatal transports, and strongly prefer CCRN or CEN at application. Add the program's own flight academy — altitude physiology, aircraft egress, survival, crew resource management — shaped by CAMTS accreditation standards.

They usually fly as a pair, and their scopes deliberately overlap in the air. The flight nurse comes up through hospital critical care — ICU drips, ventilator management, blood administration — while the flight paramedic (FP-C certified) comes up through prehospital 911 work — scene management, extrication, field airways. The nurse-paramedic duo is the standard crew model precisely because the two backgrounds cover each other's blind spots on both scene calls and interfacility transports.

Helicopter EMS carries genuine occupational risk — weather, night operations and terrain have caused fatal crashes across the industry — which is why safety infrastructure dominates the field: CAMTS accreditation standards, night-vision goggles, terrain-awareness systems, and a culture where any crew member, nurse included, can decline a flight for safety without justification.
