It suits nurses who want autonomy and a schedule they largely control, and who prefer seeing patients in context rather than in a hospital bed. The honest downsides are documentation volume that regularly spills past the workday, unpredictable driving and weather, per-visit pay that punishes slow weeks, safety considerations in unfamiliar neighborhoods, and professional isolation. BLS publishes no home health nursing wage; the containing occupation is Registered Nurses (SOC 29-1141), with a May 2025 median of $97,550 a year, $46.90 an hour, and a $68,940 to $137,470 spread. Home health agency pay generally tracks near or a little under that median, and the pay structure matters as much as the rate: per-visit compensation rewards efficient nurses with dense territories and penalizes low-census periods, while salaried roles trade that upside for predictability.

Licensure and scope. The home health nurse is a licensed RN performing skilled nursing — assessment, wound and IV care, medication management, patient teaching — and writing the plan of care that governs everyone else on the case. The home health aide provides personal care such as bathing, dressing and transfers under that nurse's plan, after a training program with a federal 75-hour minimum at certified agencies. The nurse supervises the aide, including required supervisory visits.
