How to get a paramedic license

Paramedic programmes require current EMT certification and usually a period of field experience. Time on an ambulance before starting paramedic school makes the clinical content considerably easier to absorb.

Sit the computer-adaptive cognitive examination covering advanced airway management, cardiology and resuscitation, trauma, medical and obstetric emergencies and EMS operations, and complete psychomotor competency verification through your programme or the state.

Submit the state application with your national certification and background check to obtain the credential that actually authorises practice. Some states run their own examinations instead of or alongside NREMT.

Complete your service's orientation to its protocols and obtain medical director authorisation, keep ACLS and PALS current, and recertify on both the NREMT and state cycles — the dates do not always align.

The paramedic license exam
The NREMT paramedic cognitive examination is computer-adaptive and delivered at Pearson VUE, ending when competence can be determined rather than at a fixed length, with no percentage score. It covers airway, respiration and ventilation including advanced airway management, cardiology and resuscitation including rhythm interpretation and pharmacology, trauma, medical and obstetric emergencies, and EMS operations, applied across adult and paediatric patients. Psychomotor competency is verified through the accredited programme or the state rather than a single national practical examination. Because paramedic certification requires programme accreditation, candidates cannot qualify through an unaccredited course however good the teaching.
How long it takes to get licensed
Two to three years from starting as an EMT: field experience, then one to two years of accredited paramedic education with clinical and field internships.

Paramedic license reciprocity between states
NREMT paramedic certification is the practical vehicle for moving between states: because most states build their credential on it, a paramedic with current national registration can usually obtain licensure elsewhere by application rather than re-examination. It remains an application, with the new state's background check, fee and often a protocol or jurisprudence orientation. Two further points matter more at paramedic level than at EMT: states differ on scope, and every new employer's medical director must authorise you before you practise under their protocols, which is a separate process from licensure.
