Two to three years from a standing start. The EMT course takes one semester (about 150–200 hours) plus the NREMT-EMT exam; most paramedic programs then require six months to a year of ambulance experience before admission; and the paramedic program itself runs 1,200 to 1,800 hours across one to two years, including hospital clinicals and a team-lead field internship. The NREMT paramedic exams and state licensure finish the sequence.

EMTs deliver basic life support: CPR, oxygen, bleeding control, splinting and assisting patients with a limited set of medications. Paramedics carry the advanced scope — endotracheal intubation, IV and intraosseous access, several dozen medications on standing orders, 12-lead ECG interpretation, manual defibrillation, cardioversion and pacing, and needle decompression of a collapsed lung. The BLS OEWS May 2025 median for paramedics (SOC 29-2043) is $60,600 a year, or $29.13 an hour. Private-service medics start near the 10th percentile of $44,230, while fire-based medics, field training officers and critical-care transport crews climb toward the 75th percentile of $73,470 and the 90th at $84,850. Overtime structures on 24-hour schedules mean real W-2 income frequently runs well above base — and fire-medic pensions change the lifetime math entirely.

Honest answer: the work is physically and emotionally taxing — lifting, night shifts, pediatric calls that stay with you — and burnout is the field's known occupational hazard. What makes it sustainable for many is progression: fire service (better pay, pension, staffing), flight and critical-care transport, community paramedicine, EMS education or supervision. Many others use medic years as a launchpad into nursing bridges, PA school or medicine, where the experience is gold. Plan the second chapter early.

The National Registry of Emergency Medical Technicians is the national certifying body: its paramedic credential (NRP) requires graduating from a CAAHEP-accredited program, an adaptive computer-based cognitive exam and scenario-based psychomotor evaluation. The large majority of states license paramedics directly from National Registry certification, and reciprocity between states usually rides on it, so maintaining the NRP through recertification keeps your licence portable across most of the country.

Flight paramedics staff helicopter and fixed-wing EMS crews, managing ventilated, drug-infused critical patients during transfers and scene flights. Programs typically require three to five years of busy 911 ALS experience, then the FP-C exam from the Board for Critical Care Transport Paramedic Certification, plus advanced courses in ventilator management, balloon pumps and blood products. It is the profession's most competitive niche — and its pay sits at the top of the $84,850-plus decile.
