They review inpatient charts while the patient is still admitted, looking for gaps between the clinical picture and what the record actually states — an unnamed sepsis, a heart failure without type or acuity, a malnutrition supported by findings but never diagnosed. When they find one they send the physician a compliant, non-leading query with the clinical indicators. They also educate medical staff, support denials and appeals, and work with quality on mortality and patient-safety indicator reviews.

There are two doors. Most specialists are registered nurses with several years of medical-surgical or critical care experience who learn ICD-10 coding, DRG assignment and query compliance. The other route is health information management: an associate or bachelor's degree with an RHIT or RHIA, often plus inpatient coding certification, then clinical training on disease processes. Either way hospitals typically hire from a related internal role and train on their CDI software and query standards.

None is legally required, but two credentials define the field. AHIMA offers the CDIP, Certified Documentation Integrity Practitioner, and ACDIS — the Association of Clinical Documentation Integrity Specialists — offers the CCDS, with a separate outpatient version. Both require a qualifying background (an RN license, a coding or health information credential, or a relevant degree) plus documented CDI experience, usually about two years. Employers often pay for the exam and add a differential.

For experienced nurses it is one of the more attractive exits from bedside work: weekday hours, no lifting, largely remote, and analytical work that still uses clinical reasoning. For health information professionals it is a step up in both pay and complexity from straight coding. The limits are real too — it is a small field with about 14,200 annual openings in the containing occupation, advancement means moving into management or auditing, and the physician-relations side requires diplomacy that not everyone enjoys.

BLS has no CDI-specific wage line and maps the role to Medical Records Specialists (SOC 29-2072), whose May 2025 median is $51,140 a year, $24.59 an hour, with a $37,000 to $81,150 range. That category is dominated by general health information and coding roles, so it understates CDI: specialists are concentrated near and above its 75th percentile of $64,820, and RNs moving into CDI usually negotiate against their nursing salary rather than the medical records scale.

Timing and direction. Coders work after discharge, assigning ICD-10-CM and PCS codes to what the record says, and they may not add clinical interpretation. CDI specialists work concurrently, during the admission, and their job is to improve what the record says before it is coded — by querying the physician when documentation does not reflect the clinical evidence. The two functions reconcile at the end, and disagreement between the CDI working DRG and the final coded DRG is itself a tracked metric.
