About four to six months from enrollment to employment. The state-approved course itself runs roughly 150–200 hours — commonly one semester at a community college, or an accelerated eight-to-ten-week format — followed by the NREMT cognitive exam, a psychomotor skills evaluation and state certification paperwork. Some fire academies and EMS agencies run even faster boot-camp formats for their own hiring pipelines.

The BLS OEWS May 2025 median for EMTs (SOC 29-2042) is $44,470 a year, or $21.38 an hour. Private transport companies pay near the 10th percentile of $34,130, while county services, fire-based positions and coastal markets push toward the 75th percentile of $50,100 and the 90th at $62,500. Overtime is widely available and most working EMTs out-earn their base rate; the bigger raises, though, come from promoting to AEMT or paramedic.

The EMT scope is basic life support: patient assessment, CPR and AED use, oxygen and bag-valve-mask ventilation, oral and nasal airways, bleeding control and tourniquets, splinting and spinal motion restriction, assisting childbirth, and administering or assisting with a short medication list — epinephrine auto-injectors, naloxone, aspirin, oral glucose and bronchodilator assists, varying by state. Invasive skills such as IVs, intubation and cardiac drugs belong to the AEMT and paramedic tiers above.

It is arguably the best value credential in healthcare for testing the waters. One semester in, you know whether emergency medicine suits you; the field hours count as documented patient contacts for paramedic, nursing, PA and medical school applications; fire departments treat the certification as a near-prerequisite; and hospitals hire EMTs as ER technicians. The pay is modest, which is precisely why most EMTs treat the card as a two-to-four-year platform rather than a terminal career.

Two parts. The cognitive exam is a computer-adaptive test of roughly 70 to 120 questions covering airway, cardiology, trauma, medical emergencies and EMS operations — it ends once the algorithm is confident you are above or below the passing standard. The psychomotor portion is a hands-on skills evaluation — patient assessment, ventilation, cardiac arrest management and immobilization — administered through your course or state. Pass both and you hold national certification that most states convert directly into a state card.

The Advanced EMT sits between EMT and paramedic. Building on the EMT course, an AEMT program adds a few hundred hours covering IV and intraosseous access, supraglottic airways, and an expanded medication list including dextrose, nitroglycerin and nebulized bronchodilators. It is a popular upgrade in rural systems that cannot staff paramedics on every truck, and it earns a pay differential — but full ALS practice, intubation and the wide drug box still require paramedic school.
