They care for adult inpatients with a mix of medical conditions and surgical recoveries, typically five or six at a time: assessments each shift, large medication passes, IV therapy and antibiotics, post-operative wound, drain and ostomy care, pain management, mobility and fall prevention, glucose management, discharge teaching, and constant communication with physicians, therapists and case management. The defining skill is prioritization — deciding, all shift, which of six competing demands actually cannot wait.

It remains the most common and best-supported first job in nursing, for a reason: the patient mix exposes new graduates to nearly every condition, medication class and procedure the hospital sees, and it builds assessment speed that no other setting teaches as broadly. The Medical-Surgical Nursing Certification Board, the certifying arm of the Academy of Medical-Surgical Nurses, requires an active RN license, generally two years of practice and around 2,000 hours of medical-surgical experience within the preceding three years, and a passing score on the CMSRN examination. Renewal runs on a five-year cycle through continuing education or retesting. ANCC's MEDSURG-BC is an equivalent alternative, and most employers accept either for clinical-ladder credit.

There is no separate BLS figure; med-surg nurses fall inside Registered Nurses (SOC 29-1141), whose May 2025 median is $97,550 a year, $46.90 an hour, from $68,940 at the 10th percentile to $137,470 at the 90th. Because med-surg is the largest and most entry-heavy segment of that occupation, its typical pay sits at or slightly below the median, with differentials, certification, clinical-ladder tiers and premium incentive shifts accounting for most of the upward movement.

There is no national standard. California sets a legal maximum of five patients per nurse on medical-surgical units, and a handful of other states have staffing-committee or disclosure laws rather than fixed numbers. In practice assignments of five to seven are common, and higher on nights. Because acuity on these units has risen as lower-acuity care moved outpatient, ratio is the specialty's central workplace issue and a major driver of both turnover and union activity.

Yes, and many nurses do. Clinical ladders reward certification, precepting, evidence-based practice projects and committee work with real pay tiers, and experienced med-surg nurses become charge nurses, unit educators, preceptors for new residents, informatics analysts or clinical nurse leaders without leaving the specialty. The alternative use of the same experience — as the prerequisite for ICU, ED, CRNA or nurse practitioner tracks — is equally valid, and the choice usually arrives around year three.
