Florida physical therapists earn a median $101,040 a year, $48.58 an hour, per BLS OEWS May 2025, slightly below the national figure but with no state income tax withheld. The middle half earns between $83,650 and $107,520, and the full range is $73,630 to $122,880. Setting rather than seniority explains most of that spread.

Naples-Marco Island leads at $105,440, then Port St. Lucie at $105,240, North Port-Bradenton-Sarasota at $104,020, Homosassa Springs at $103,510 and Tampa-St. Petersburg-Clearwater at $102,430. Four of the five are retirement-dense markets, which is where Florida's rehabilitation volume is concentrated.

Graduate from a CAPTE-accredited DPT programme, pass the National Physical Therapy Examination and the Florida laws and rules examination, and apply to the Florida Board of Physical Therapy Practice within the Department of Health. Therapists licensed elsewhere apply by endorsement but still complete the Florida laws and rules requirement.

In a limited form. A Florida-licensed physical therapist may evaluate and treat a patient without a referral subject to statutory conditions, including time limits after which physician involvement is required and additional requirements for certain therapists. It supports outpatient demand but does not create the unrestricted direct access some states allow.

Pennsylvania at $101,380, South Carolina at $101,330, Michigan at $100,900, Kentucky at $100,710 and New York at $100,660 all sit within about a thousand dollars of Florida on gross. Florida's advantage over that group is the absence of a state income tax; its disadvantage is housing and insurance cost in the coastal counties.

Because they generate the case mix that supports higher-paying settings. Naples-Marco Island, North Port-Bradenton-Sarasota and Homosassa Springs have concentrations of skilled nursing, home health and post-acute rehabilitation that younger markets do not, and those settings pay above outpatient orthopaedics. It is a case-mix effect rather than a cost-of-living one, which is why Homosassa Springs sits alongside Naples.

Productivity expectations and payer mix. A patient population weighted toward Medicare means tight documentation requirements and utilisation review, and outpatient clinics respond by raising visit volume. Therapists coming from states with a younger commercial payer mix consistently report the paperwork and the pace as the adjustment, more than the pay.

Anchor on the 25th percentile of $83,650 as the realistic first-year figure, not the median, and weigh it against DPT debt with the tax advantage included. Florida's arithmetic works better than the gross ranking suggests, but it works considerably better inland and in the Tampa corridor than in the Gulf coast markets where housing and insurance have risen fastest.
