California EMTs earn a median $46,380 a year, $22.30 an hour, per BLS OEWS May 2025. The middle half earns between $42,610 and $59,180, and the full range is $38,740 to $78,150. The bottom of that range is private ambulance work and the top is fire-based or public-agency employment, which is the single biggest determinant of EMT pay in this state.

Redding leads at $60,120, which reflects rural fire-based EMS and the difficulty of staffing far northern California. San Jose-Sunnyvale-Santa Clara follows at $54,650, then Santa Cruz-Watsonville at $51,970, San Francisco-Oakland-Fremont at $51,480 and Sacramento-Roseville-Folsom at $49,550. Note that the highest-cost metros are not the highest-paying ones.

Complete a state-approved EMT course, pass the National Registry cognitive and psychomotor exams, then apply for certification through a local EMS agency. California runs EMT certification through those county-level agencies under the authority of the state Emergency Medical Services Authority, and you also need separate accreditation from the agency in each county you want to work in.

In most of the state, not on its own at the median. That is why the realistic California path treats EMT as a paid entry credential: EMTs typically run one to three years of field time and then move to paramedic school, a fire academy or nursing, all of which are gated on that field experience. EMTs who instead move into fire-based or hospital employment reach the state 75th percentile of $59,180 without changing scope.

Different funding. Private providers are paid per transport under a county contract and compete on price at bid time, so labour cost is the variable they control. Fire districts and hospital systems pay on a public salary schedule with a pension, and staff EMS from the same budget as the rest of the department. The scope of practice is identical; the employer is what changes.

Because the private ambulance sector, which employs most California EMTs, is priced by county transport contracts rather than by the local cost of labour. Those contracts are competitively bid, so wage pressure that shows up in other California occupations does not pass through. The occupations that sit next to EMT in a hospital or fire department do carry the California premium, which is why the exit rate out of private EMS here is so high.

It changes employer access as much as scope. Paramedic certification opens fire-department and critical-care roles that EMT certification does not, and California paramedics earn a median well above the top of the EMT range. If you are looking at this page while deciding whether to stay an EMT, the honest answer is that the paramedic or fire route is where the pay progression is.

More than newcomers expect. State certification is portable, but each local EMS agency accredits separately, with its own protocols, orientation and sometimes its own testing. Moving from one county to another can mean weeks of re-accreditation before you can work, which suppresses the wage competition between neighbouring counties that would otherwise lift pay.
