Pennsylvania EMTs earn a median $43,310 a year, $20.82 an hour, per BLS OEWS May 2025, with the middle half between $37,630 and $47,480 and a full range of $34,480 to $54,370. That sits 2.6% below the $44,470 national median, despite Pennsylvania employing far more EMTs per worker than the US average.

Philadelphia-Camden-Wilmington at $47,790, then Lancaster at $44,200, Reading at $44,100, Pittsburgh at $43,240 and Lebanon at $42,440. Outside Philadelphia the table is remarkably flat — about two thousand dollars covers the other four — so within Pennsylvania, moving agencies rarely changes pay much unless you move to Philadelphia.

Through the Bureau of Emergency Medical Services at the Pennsylvania Department of Health, administered via the state's regional EMS councils. You complete an approved EMT education programme, pass the National Registry cognitive and psychomotor examinations, and apply for Pennsylvania certification. It renews on a three-year cycle with continuing education, and the same system administers the Emergency Medical Responder, Advanced EMT and Paramedic levels.

Geography and history. Pennsylvania is a large state with a lot of rural and small-town territory, and it built emergency medical coverage through a dense network of community, municipal and volunteer-rooted agencies rather than a small number of large services. That produces a location quotient of 1.44 across about 10,160 EMTs — a lot of people, spread across a lot of small employers, which is precisely the structure that does not push wages up.

They take a proportionally meaningful bite. The state rate is a flat 3.07%, but municipal and school district earned income tax adds commonly around one per cent, and Philadelphia's city wage tax is substantially higher — so part of the Philadelphia premium of $47,790 over the state median of $43,310 goes to the city. At this income level those percentage points are worth paying attention to when comparing offers across county lines.

Because the concentration reflects fragmentation. Pennsylvania's EMS is delivered by a very large number of small municipal, community and hospital-affiliated agencies rather than by a few large employers, and small agencies with limited local tax bases and heavy reliance on transport reimbursement cannot bid wages up. It is an annual wage in a field where hours vary enormously, and where holding positions at two agencies is common.
