About two years. The standard route is an associate degree from a CoARC-accredited program with hospital clinical rotations built in, followed by the NBRC's TMC exam and Clinical Simulation Examination — most graduates complete both within months of finishing school — and then state licensure. Bachelor's programs take four years and matter mainly for future management or teaching roles; bedside scope and starting pay are essentially the same either way.

Both come from the NBRC's exam sequence. Scoring above the low cut on the Therapist Multiple-Choice exam earns the CRT (Certified Respiratory Therapist); scoring above the high cut qualifies you for the Clinical Simulation Examination, and passing that confers the RRT (Registered Respiratory Therapist). The RRT is the advanced credential and the practical hiring bar — most hospitals now require it for ICU positions, and many post every RT job as RRT-only.

Scope depends on state law and hospital privileging. Everywhere, RTs manage ventilators, assist intubations, and run bag-valve-mask ventilation during codes; in many hospitals — especially transport, rural and some academic centers — credentialed RTs perform intubation themselves under protocol. RTs also routinely place arterial lines for blood gases in some systems. The airway is the RT's territory in an emergency, whoever passes the tube.

BLS OEWS May 2025 reports an $82,280 national median — $39.56 an hour — for SOC 29-1126. New grads start near the 10th percentile of $63,660; experienced ICU therapists with ACCS or NPS credentials reach the 75th percentile of $98,730; and the top 10%, typically leads, ECMO specialists and travel RTs in high-cost markets, exceed $118,050. Night and weekend differentials make actual hospital take-home run above quoted base rates.

It is acuity-dependent in a way few careers are. A floor-therapy shift of nebulizers and oxygen rounds is steady, protocol-driven work; an ICU or ED shift means ventilator emergencies, codes and end-of-life decisions, sometimes back to back. RTs carry the airway pager for the whole hospital. Twelve-hour shifts and night rotations are standard. People who find crisis competence energizing tend to thrive; the specialty ladder also lets you choose your acuity as your career matures.

Strong: BLS projects 12.1% employment growth from 2024 to 2034 — well above the all-occupation average — with roughly 8,800 openings a year. The demand base is demographic and durable: COPD and chronic respiratory disease climb with age, critical-care units have expanded, and home ventilation and pulmonary rehab keep growing care outside hospitals. Pandemic experience also permanently raised health systems' baseline investment in ventilator-competent staff.
