You care for patients who need continuous cardiac monitoring but not intensive care — post-heart-attack, heart failure, arrhythmia, post-catheterization and post-device patients on a typical four-to-six patient assignment. Beyond standard medical-surgical nursing you interpret rhythm strips throughout the shift, run twelve-lead ECGs, titrate cardiac drips to protocol, monitor sheath sites, and act as the early-warning system that escalates deteriorating patients before they arrest.

Complete an ADN or BSN from a CCNE- or ACEN-accredited program, pass the NCLEX-RN and get your state license, then hire directly into a telemetry or progressive care unit — these floors take new graduates and support them with 10 to 16 weeks of orientation. It is demanding but well supported and widely considered one of the strongest first jobs in nursing. The challenge is combining a heavier patient load than ICU with genuine acuity and the constant cognitive load of watching rhythms while doing everything else. The payoff is that telemetry builds cardiac assessment, drip titration and escalation judgment faster than a general medical-surgical floor, which is why ICU, cath lab and emergency departments recruit heavily from step-down.

Acuity and ratios. ICU nurses typically carry one or two patients who are often ventilated, on multiple vasoactive drips and requiring invasive hemodynamic monitoring. Telemetry nurses carry four to six patients who are monitored and unstable enough to need watching but stable enough to be off intensive care. Both use cardiac rhythms and drips; the ICU role goes deeper on fewer patients, while telemetry demands triage across a bigger board. AACN certifies them separately — CCRN versus PCCN.

BLS does not publish a telemetry-specific figure. Registered Nurses (SOC 29-1141) have an OEWS May 2025 median of $97,550 a year ($46.90/hr), ranging from $68,940 at the 10th percentile to $137,470 at the 90th. Telemetry nurses generally track close to the general RN band, with night and weekend differentials, PCCN certification pay, charge duties and travel contracts accounting for most of the movement above the median.

Telemetry is one of nursing's best-connected specialties. The common next steps are ICU, cardiac catheterization lab, electrophysiology, emergency department and rapid response — all of which value the rhythm and drip fluency the unit builds. Others move laterally into cardiac device or heart-failure clinic roles, or upward into charge nurse and step-down unit management. Graduate paths include acute care nurse practitioner and cardiac clinical nurse specialist.
